About knee pain

Common causes of knee pain

Most knee pain in active and working adults has a mechanical cause that responds well to a structured rehabilitation approach once properly assessed.

Patellofemoral pain

Pain around or behind the kneecap, often worse on stairs, squatting, or after prolonged sitting. Typically load and hip control related, and responds well to progressive rehabilitation.

Patellar and quadriceps tendinopathy

Pain at the front of the knee with loading activities such as running or jumping. Progressive tendon loading is the most effective approach and is incorporated into the programme from the first session.

ITB syndrome and runner's knee

Lateral knee pain that builds during running. A combination of load management, hip strengthening, and hands-on treatment usually resolves it within a structured programme.

Treatment approach

What treatment for knee pain involves

Assessment includes movement screening, orthopaedic tests, and a review of training load or daily activity. Treatment combines hands-on work where appropriate with a progressive exercise programme targeting the specific structures involved. Load management advice is provided from the first session.

  • Orthopaedic assessment to identify the pain source
  • Progressive loading programme matched to your condition
  • Training load and activity modification advice
  • Hip and lower limb control work where relevant
Self-Help

Between sessions

See the exercise library for knee starting points including quad sets, sit-to-stand, and straight-leg raises. These are general suggestions; your specific programme will be agreed at assessment.

Manage load, not rest

In most knee conditions, reducing aggravating load is more useful than stopping all activity. Walking and low-load movement are usually well tolerated.

Ice and compression

For an acutely irritated knee, short periods of ice wrapped in a cloth and gentle compression can reduce swelling and discomfort while awaiting assessment.

Red flags

Significant swelling after an injury, inability to weight-bear, or a knee that locks requires urgent assessment. See the red-flag check.
